This innovative program is available to both full-time and part-time associates

* Less administrative paperwork As a Pay Per Visit Physical Therapist, you
will:
*Test/screen the patient's physical strengths to assist the physician in evaluating the patient's
level of function and record findings to develop or pursue treatment programs and establish measurable
training objectives.
*Develop/implement a conditioning/rehabilitation program consistent with physician's
Plan of Treatment and the overall goals of the patient/rehab team. Adjust treatment as needed to achieve
maximum results.
*Confer with physician and other team members to obtain additional patient information
and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.

*Integrate physical therapy with other aspects of patient's healthcare and training needs in accordance
with physician's orders. Participate in family education meetings. Qualifications
*Degree from an
accredited Physical Therapy Program (approved by the American Physical Therapy Association, the Committee on
Allied Health Education and Accreditation of the American Medical Association and/or the American Physical
Therapy Association) and meet any additional qualifications as stated in state homecare regulations
*Six
months of experience
*Current licensure/certification in the state of practice and current CPR
certification
*Good organizational and communications skills expect more from Total Care Home Health
